Pediatric status epilepticus: How common is cerebrospinal fluid pleocytosis in the absence of infection?

چکیده
PURPOSE To determine the rate of cerebrospinal fluid (CSF) pleocytosis among children presenting with status epilepticus (SE) without proven central nervous system infection. METHOD We performed a retrospective cross-sectional study of all patients aged one month to 21 years of age who were evaluated in a single pediatric emergency department (ED) for SE between 1995 and 2012. We limited our study to those children who had a CSF culture obtained and excluded those children with proven viral or bacterial infection. We defined SE in a patient who had a single seizure or a cluster of seizures without regaining consciousness which lasted 30 min or longer. We defined CSF pleocytosis as a CSF white blood cells (WBC)>10 cells/mm(3) and a peripheral leukocytosis as WBC ≥ 15,000 cells/mm(3). We compared the rate of CSF pleocytosis between children with and without peripheral leukocytosis using the Fisher's exact test. RESULTS We identified 289 ED visits for SE, of which 178 (62%) met study inclusion criteria. Seven children (4%, 95% confidence interval 1.7-8.2%) had CSF pleocytosis. More children with peripheral leukocytosis had CSF pleocytosis: (8.6% with peripheral leukocytosis vs. 0.9% without leukocytosis, p=0.01). CONCLUSION CSF pleocytosis is relatively uncommon among children with prolonged seizures, even in the presence of peripheral leukocytosis. Therefore, all children with CSF pleocytosis after status epilepticus need comprehensive evaluation for central nervous system infection.

Fungal infections of the CNS - Section 1
While fungal infections of the CNS are relatively rare, they have become more common with the increasing number of individuals who are immunocompromised due to HIV/AIDS, immunosuppressive therapies, invasive diagnosis and treatment methods, and organ transplants. CNS fungal infections present many diagnostic and therapeutic challenges and are associated with a high mortality rate. Cryptococcal ...
متن کامل844The Clinical Significance of Cerebrospinal Fluid Neutrophilic Pleocytosis in Viral Central Nervous System Infections
Background. Viral meningitis and encephalitis is typically characterized by a cerebrospinal fluid (CSF) lymphocytic pleocytosis. A CSF neutrophilic pleocytosis presentation has been described but its prognostic and clinical significance is unknown. The objectives of our study were to compare the clinical and laboratory characteristics of viral central nervous system (CNS) infections with CSF ne...
متن کاملA Giant Abdominal Cerebrospinal Fluid Pseudocyst
Background and Aim: Ventriculoperitoneal shunt (VPS) is the most commonly used procedure for treatment of hydrocephalus in children. Abdominal cerebrospinal fluid pseudocyst is a rare complication but potentially fatal. The pathogenesis of this complication remains unclear. This complication should be considered during differential diagnosis of an abdominal mass. Our aim is to report how large ...

INTRODUCTION To characterise a homogeneous group of patients with new-onset refractory status epilepticus (NORSE syndrome). MATERIALS AND METHODS This is a descriptive, semiprospective review of all cases of NORSE syndrome seen between 2000 and 2004 at a tertiary care public hospital in Singapore. A review of the literature was performed to identify possible additional similar cases for compa...
متن کاملCerebrospinal fluid pleocytosis level as a diagnostic predictor? A cross-sectional study
BACKGROUND Lumbar puncture with quantification of leukocytes and differential count of cellular subsets in the cerebrospinal fluid is a standard procedure in cases of suspected neuroinfectious conditions. However, a number of non-infectious causes may result in a low leukocyte number (0-1000 cells/ml). We wanted to assess the diagnostic diversity of unselected adult patients with pleocytosis in...
